Federal Work Study Authorization Form

    Instructions:  Once you’ve been offered an on-campus position, you must:

    1. Fill out the first two sections of this form (Student & Department Information). If you are unsure of an answer, please contact your direct supervisor. 
    2. Once you have been completed and submitted the form, you will need to meet with the Financial Aid Office to review your Financial Aid package. 
    3. You may either book an appointment online (select general session), call and schedule a time to meet with a Financial Aid Counselor, or walk-in appointments are welcome M-F 8:30AM - 4:30PM (Bamberger Hall, Room 204.  Once the steps have been completed, the Financial Aid Office will submit your information to the Career Center to finalize the hiring process. 
    NOTE:  You cannot begin working on campus until this form has been completed and submitted. You only need to complete this form once every academic year, regardless of how many positions you are hired for on campus.
